Issuance of Identification Cards

The Registrar’s Office issues identification card to all new students who are officially enrolled.

Replacement of ID can be effected in cases of:

1. Lost ID.
2. Damaged ID.

Lost ID/Replacement

Students who apply for ID replacement must present:

1. Clearance Form from the Discipline Unit.
2. Official Receipt .
